  • Dementia or cognitive impairment
  • Mental health conditions
  • Pain, fear, or disorientation
  • Substance intoxication or withdrawal
  • Brain injury affecting impulse control
  • Frustration with wait times or perceived poor treatment

The Security Officer's Role in Aggression Management

In a well-designed healthcare security program, security officers are not the first response to patient aggression — they are one part of a system that includes clinical staff, social workers, and management. The security officer's role is typically to:

  • Support clinical staff who are managing an agitated patient
  • Intervene physically only when there is an immediate risk of harm to staff or other patients, and only using the minimum force necessary

Physical intervention in healthcare should be the last resort — not because of squeamishness, but because physical intervention with patients creates clinical risks (injury, psychological trauma, impact on therapeutic relationship) that don't exist in other security contexts.

De-escalation is the primary tool, and healthcare security officers require specific training in:

  • Trauma-informed communication
  • Dementia-specific communication techniques
  • Mental health first aid
  • Recognising and responding to specific clinical presentations
  • Collaborative approaches with clinical staff

This training is significantly beyond the standard security guard qualification and must be specifically selected for in healthcare security officers.

Environmental Design and Security

Environment plays a significant role in aggression in healthcare settings. Security teams should assess and advise on:

  • Waiting area design — long waits in uncomfortable, crowded, noisy spaces increase agitation. Comfortable, well-managed waiting areas reduce aggression at the source.
  • Signage and communication — clear, respectful signage about wait times and processes reduces frustration-driven aggression.
  • Duress systems — staff personal duress alarms, panic buttons at reception desks, and code systems allow rapid security response without escalating patients.
  • Sightlines and CCTV — security officers need to see the full waiting area and triage zone without entering the clinical space unnecessarily.
  • Entry control — managed entry to the ED or acute areas prevents the introduction of outside aggressors while allowing patient family access.

When to Physically Intervene

Physical intervention by security in healthcare is appropriate when:

  • A person is actively assaulting staff or patients and de-escalation has failed
  • A person is immediately threatening serious harm
  • Clinical staff have requested assistance with a patient who poses a risk

Any use of force must be documented in an incident report, reviewed by security management, and reported through clinical incident management systems. Healthcare facilities with high rates of physical intervention should review whether environmental or system factors are driving preventable escalations.

Building the Right Healthcare Security Team

Healthcare security requires officers who are specifically suited to the environment — patient-focused, calm under pressure, capable communicators, and genuinely committed to the hospital's mission of care. The wrong personality in a healthcare security role creates more problems than it solves.
